内視鏡的胃粘膜切除術

我々は,関門医療センター消化器科において,内視鏡的粘膜下層剥離術(ESD法)導入初期の2004年4月から11月までの8か月間に,連続する胃上皮性病巣43病巣を,内視鏡的に加療した.切除法の内訳は,Strip biopsy法35病巣,ESD法8病巣である.この間,偶発症として,顕出血1例(2.3%),穿孔・穿通3例(7.0%)が経験された.穿孔・穿通を生じた要因は,過大な切除範囲の設定・強引な切除・盲目操作であった.これら穿孔・穿通の3例は,内視鏡的クリップ閉鎖・絶食・抗生剤投与にて,保存的に加療し得た.
